Pricing

We are an “out-of-network” provider and do not file directly with insurance companies for reimbursement. We will provide you with a “superbill,” or receipt of services provided and payment, at the completion of the evaluation that you may choose to submit to your insurance carrier to seek reimbursement.

Evaluation fees vary depending on referral questions and hours of work needed to answer the referral questions. A detailed Testing Fee Agreement and Good Faith Estimate (GFE) will be provided following the initial interview and before testing begins to ensure clarity regarding all billing and payment expectations. The hourly rate for testing and related activities (such as scoring, report writing, record review, etc.) is $250/hour, and the cost of a full evaluation typically ranges from $1600 to $4500+.

  • We accept the following forms of payment for evaluations:

    • Major credit cards (Visa, MasterCard, American Express, Discover)

    • Debit cards

    • Paper checks payable to New Spark Psychology & Consulting PLLC

  • The fee for the intake/diagnostic interview shall be paid the day of the initial appointment. Half of the remaining estimated total fee shall be paid on the day of the first testing appointment. The remaining portion of the total fee shall be paid either on the final day of testing, or upon notification to the client prior to report completion, as specified in the GFE.

  • Payment plans are available for non-forensic evaluation services on an as-needed basis. If you need assistance spreading the cost, please let us know when you schedule. We will discuss available options and create a plan that fits your circumstances.
